How to Request Your Medical Records
Under Texas law and federal HIPAA standards, you or your authorized legal representative have the right to review, inspect, and receive copies of your clinical records. Submit your request using any of the methods below.

Send a written request with the subject line "Medical Record Request". Include your full name, date of birth, and records date range.

Required Identifiers
Your written request must clearly specify your full legal name, date of birth, contact phone, exact clinical dates or records requested, and preferred format (electronic PDF or paper).
15 Business Day Standard
Per Texas Health & Safety Code regulations, electronic health records are fulfilled and delivered within 15 business days following receipt of your signed authorization.
Zero Retrieval Fees
You are never charged a records retrieval or search fee. Only reasonable, actual cost-based fees for paper duplication or postal delivery apply if physical copies are requested.
