How to Request Your Medical Records

You have the right to review and obtain a copy of your health records. To request your medical records from Vertrees Headache Center, please submit a written request using one of the following methods:

Your request should include:

Your full legal name, date of birth, contact information, a description of the records you are requesting, and your preferred format (electronic or paper).

Response Time

If we maintain an electronic health record system, your records will be provided within 15 business days of receiving your written request.

Fees

You may be charged a reasonable, cost-based fee for copying and mailing your records. You will not be charged a retrieval fee.
